What is the County Health Improvement Plan?
The goal of the County Health Improvement Plan (CHIP) is to develop and implement a five-year county plan to improve health and achieve health equity by positively turning the indicators for each of the health priorities.

What is a County Health Assessment?
Alameda County conducted a County Health Assessment (CHA) to gain an understanding of core health challenges and health and social inequities in Alameda County. Our sources for this process included the health department, local governments, community-based organizations and collaboratives.
After completing the review of 49 prior community assessments, a summary of the overall findings identified several key factors:
- a need for greater emphasis on the use of data and intervention measurement
- to hear from and engage hidden community voices.
- the need to develop strategies that streamline work and partner involvement and greater accountability for implementing recommendations.

The assessment and its findings led to these three County Improvement Priorities:
- Accessible medical and mental health care that is high quality, comprehensive, affordable, culturally and linguistically appropriate and supports good health and the expectation for all to live a full and productive life.
- Community economic development that supports the ability of all residents, regardless of race/ethnicity or place of residence, to pay for all of their basic needs (including housing, food, transportation, healthcare, and childcare) and build wealth.
- Community and Public Safety that prevents crime and abuse, and ensures neighborhood safety and community resilience to disasters and emergencies.

Why was the County Health Improvement Plan (CHIP) Advisory Committee established and what is its goal?
After conducting the assessments, the County Health Improvement Plan (CHIP) Advisory Committee was established to work with the Alameda County Board of Supervisors and population-specific community-based organizations to convene a series of 25 focus groups to present population-specific data and to gather input from communities to establish priorities for improving health.
