Foster Care

Health Care Program for Children in Foster Care (HCPCFC) addresses the health needs of foster children. Public Health Nurses (PHNs) work with Child Welfare Workers and Probation Officers to gather health information (medical, dental and behavioral) on each foster child to create a Health and Education Passport. This Passport travels with the foster child to the foster care provider so that health care is continuous for the child.

Program Eligibility

• all children in the Child Welfare system or Juvenile Probation system
• all children designated by the courts to be foster children

Useful Information

HCPCFC does the following:

• collects relevant health data on each child in foster care
• consults with child welfare workers and probation officers on health plan for each foster child
• educates foster parent, group home providers, welfare and probation workers on health needs of the child
• serves as a resource to foster parents, foster children, group home providers, child welfare workers, and probation officers
