Is Care Partners a case management program?
No. Care Partners is a non-medical and light-touch, operating like a hub to the needed resources in the community and we can connect you to case management services.
Are your services free?
Yes.
Can clients self-refer?
Yes.
If I am not an In-Home Supportive Services Care Provider or Recipient, but I reside in Alameda County, can I still get services with you?
Yes, if you meet the eligibility.
In what languages are your services provided in?
Currently, the multi-lingual team members can provide services in English, Spanish, Cantonese, Korean, Mandarin and Vietnamese. Language Line support will be used for languages other than those listed above.
Are you In-Home Supportive Services (IHSS)?
No, Care Partners works alongside with IHSS to provide support to the IHSS Care Providers and Recipients.
Can you help me apply for In-Home Supportive Services (IHSS)?
Yes.
