Our Services

Care Partners program consists of an interdisciplinary and non-medical team, operating like a light-touch hub for the community and providing FREE support in the following:

Provide compassionate care, deep listening, and emotional support to older adults.
Connections to local resources: Food, Housing, Transportation, Assessment of Eligibility for digital devices and MORE!
Assisting in obtaining durable medical equipment and safety equipment, such as a walker, grab bars and so MUCH MORE!
Educating and empowering clients to explore the kind of care they want by providing support and advocacy through navigating the complex healthcare system and transition of care.
Increase Life Care Planning and assist in completing Advance Health Care Directives.
