Community Assessment, Planning, and Evaluation (CAPE)
CAPE is situated in the Alameda County Public Health Department and serves both Public Health and Alameda County Health Care Services Agency programs and partners.
CAPE Vision
Everyone in Alameda County, no matter where you live, how much money you make, or the color of your skin, has a healthy, fulfilling, and productive life.
CAPE Mission
CAPE provides information, evaluation, planning, and technical assistance to programs, community partners, decision makers, and residents in order to improve community health and address root causes of health inequities.
We do this by
- Monitoring population health status and social determinants of health
- Presenting health and social data in a way that is impactful
- Consulting with programs about needs assessment and evaluation design
- Developing evaluation tools and strategies (e.g. surveys and focus groups)
- Managing and analyzing program data
- Providing information to support planning and policy advocacy
- Facilitating the implementation of Results-Based Accountability for measuring success
- Improving agency-wide communications
