A Public Health Nurse (PHN) is a Registered Nurse (RN) with special training in community health. A PHN can help you or your family with health concerns and parenting and lifestyle issues. PHNs are knowledgeable about the resources in your community.
A Community Health Outreach Worker (CHOW) is a paraprofessional who lives in or is familiar with the community. CHOWs are trained to provide basic health education and referrals for a wide range of services, and to provide support and assistance in navigating the health and social services system.
Public Health Nursing staff is organized into two Community Health Teams specific to a service region:
- Region I (North County) covers Alameda, Albany, Emeryville, Oakland, and San Leandro
- Region II (South County) covers Castro Valley, Dublin, Fremont, Hayward, Livermore, Newark, Pleasanton, San Leandro, San Lorenzo, and Union City
