Alameda County Health Care for the Homeless Street Health Teams
Alameda County has one of the highest rates in the country of “unsheltered homelessness” or people living in streets, RVs, encampments, etc. ACHCH works to bring health care and housing services directly to unsheltered persons through Street Health Teams, operated by ACHCH, LifeLong Medical Care, Tiburcio Vasquez Health Center, ROOTS Community Health and Bay Area Community Health to provide street medicine, harm reduction, housing and substance use services in 14 zones covering the entire county.

Outreach Zones for Alameda County Street Health:
Street Health Teams provide nurse-led health care, including primary care, preventative care, behavioral health, and substance use services ranging from Medication Assisted Treatment to harm reduction supplies and resources. Teams provide housing assessments and enrollment, and support to people living in encampments, tents, vehicles, streets and other unsheltered settings.
