Instructions for Sworn Statements by Morticians to Purchase Certified Copies of Death Certificates

Sworn statements do not replace the “Vital Registration Request for Service.” The request for service form will still need to be completed. The law states that funeral directors do not need to notarize sworn statements. Only licensed funeral directors may sign a sworn statement.

Sworn statements are needed in order to get a certified copy of a fetal demise certificate; a certified copy for a Veteran, or certified copies for an authorized person as defined by law.
